My approach is similar to Cory's. Once the players had their character backgrounds roughed out I would usually add an element or two that would have brought them to the same place (the campaign start point) at the same relative instant. It usually revolved around some sort of family obligation or other pressing tie, so they felt compelled to go. Since I have players roll for origin before doing anything else it was also usually fairly easy to ensure they were all outsiders and thus had a reasonable motive for working together at first. In a couple of cases characters came from the same region, so that helped draw them together. After that, the need to procure resources to finish their individual quests started them working as a team.

It's obviously much easier in military and espionage games. The Western ones take a bit more work, but if you have common enemies you can draw characters together just based on the starting premise of "the enemy of my enemy is my friend."
