Looking for pdf of pages cut from RMSS Mentalism Companion

When the RMSS Ment Co was reprinted for RMFRP, a couple of pages were cut. The pages were made available on the old ICE site as a free download, but I can't find this pdf now.

Could someone point me to a download location? Or I could provide my email address via PM and it could be sent.

I was so free to throw the text and pictures on 2 pages, so one could print a doublesided page and just throw it in the mentalism companion. US format page, 14mm border, give me a note if there are errors or trouble.
No copyright violation intended.
